Liability Release By signing this Release, I acknowledge that: I consent to the participation of my child with Poquoson Baptist Church and that they are physically and emotionally able to participate in their programs and activities. I certify that I have the authority to release Poquoson Baptist Church from any and all claims and I have not transferred such authority to any other person. I consent that I am signing this Release freely, voluntarily, and after sufficient review. I authorize the volunteers, staff, and elders of Poquoson Baptist Church to make emergency medical decisions on behalf of my child in the event I or those listed as emergency contacts can not be reached. I acknowledge that I will ultimately be responsible for the cost of any medical care provided to my child. I release Poquoson Baptist Church, its elders, deacons, staff, and volunteers from any and all liability for injury, loss, or damage to my child or property during the course of my child’s involvement with their programs and activities I agree to attempt to settle all claims through good faith negotiations with Poquoson Baptist Church. If good faith negotiations fail, I agree to attempt to settle all such claims through mediation with a professional mediator. If mediation fails, I agree that the laws of the Commonwealth of Virginia shall govern and the jurisdictional forum shall be the Commonwealth of Virginia.
